Who qualifies for Chronic Care Management (CCM)?

Many Medicare beneficiaries may qualify for Chronic Care Management if they have two or more chronic conditions that are expected to last at least 12 months (or for the remainder of life) and require ongoing medical management.

Examples of qualifying chronic conditions may include, but are not limited to, diabetes, hypertension, heart disease, heart failure, coronary artery disease, arthritis, osteoporosis, COPD, asthma, chronic kidney disease, chronic pain, obesity, stroke, high cholesterol, thyroid disorders, dementia, Alzheimer’s disease, Parkinson’s disease, and many other long-term medical conditions.

Qualifying behavioral and mental health conditions may also include depression, anxiety disorders, bipolar disorder, schizophrenia and other psychotic disorders, post-traumatic stress disorder (PTSD), obsessive-compulsive disorder (OCD), panic disorder, generalized anxiety disorder, substance use disorders in recovery, and other chronic behavioral health conditions when they require ongoing management and coordination of care.
